108 COMBATTING CONFUSION: SIMULATION-BASED MEDICAL EDUCATION FOR DEALING WITH DELIRIUM

Abstract Background Delirium is a complex, costly, and common clinical syndrome. Simulation-based medical education has been shown to be effective in enhancing the confidence, knowledge skills of healthcare professionals management delirium. We piloted an inter-professional simulation on delirium acute hospital setting. The session’s design was informed by participating ‘Sim Stars’, regional development competition. Methods iteratively developed low-fidelity scenario recognition, prevention resulting during hour-long multidisciplinary departmental teaching session lecture hall A candidate asked review acutely confused patient (played ‘confederate’) call. expected screen for using 4AT tool; assess possible causes delirium; implement appropriate plan. After session, qualitative feedback from participants audience sought. Results This pilot included one (medical Senior House Officer); two confederates (Consultant Geriatrician, Advanced Nurse Practitioner); facilitators 17 members. Feedback indicated that learners trainers found this helpful enjoyable pedagogical method. Areas improvement were highlighted, including providing advance notice topic simulated; more structured introduction defining learning outcomes declaring ‘blame free’ environment; improved script confederate; having team (rather than specialists) play ‘confederates’ may help improve team-building reduce learner anxiety. Conclusion received positive appraisals team, plans are underway incorporate regularly into teaching. Suggestions future simulations welcomed. Given its low-fidelity, can easily used at any site simulation-based training.
